Please fill this form out if you would like to initiate GI Bill benefits for the first time.  It is currently an outdated form, which we are working on revising.  Only fill out the first page of the document.  If you are a Post-9/11 GI Bill recipient just fill out the bottom portion of the first page and ignore the top half.

Quarterly Certification Form

If you are a continuing student who has already registered with this office and would like to continue receiving your VA Education Benefits (except Cal Vet Fee waiver), please submit the form below.  It is recommended that you submit as soon as you sign up for classes for the upcoming term.  If you are on Post-9/11 GI Bill, your form is due by the 1st of the month that there is a Fee Deadline on the 15th of that same month for the upcoming term if you would like your Tuition and Fees to be covered by the Fee Deadline.  If you submit late, it is your responsibility to ensure your tuition and fees are covered by another means, while we process, we will reimburse you at a later time.

Major/Minor Declaration Form

These Declaration forms are only used if you are between majors.  For example, the system shows you as a Biological Sciences major, but you have spoken to your counselor and will start to pursue a Public Health Policy major.  This should be turned in with your Quarterly Certification Form that reflects the classes in the new major you are pursuing.  It should also list courses that you will need to take in upcoming quarters before the major change becomes official.  Once the major change is official and reflected in your degree audit, this will no longer be needed.
